Изменение кишечной микробиоты как фактор риска развития бронхиальной астмы

Бронхиальная астма (БА) относится к числу распространенных и социально значимых заболеваний. По данным ВОЗ, заболеваемость БА неуклонно растет, составляя на сегодняшний день около 300 млн человек в мире. Профилактика БА представляет собой актуальную задачу современной медицины. Обсуждаются современные представления о роли микробиоты кишечника в патогенезе БА. К настоящему времени имеются веские доказательства взаимодействия микробиоты кишечника и респираторного тракта посредством оси «кишка–легкие». Рассматриваются факторы, влияющие на состав микробиоты кишечника, в том числе состав микробиоты материнского организма, способ родоразрешения, способ вскармливания, назначение лекарственных препаратов, а также влияние факторов окружающей среды. Проведен анализ результатов экспериментальных и клинических исследований, подтверждающих влияние кишечной микрофлоры на развитие и прогрессирование заболевания. Установлено, что нарушение состава микробиоты и ее метаболической активности способствует поляризации иммунного ответа в сторону Т-хелперов 2-го типа. В то же время выраженное биологическое разнообразие микроорганизмов кишечника способствует адекватному производству бактериальных метаболитов, индуцирует иммунные регуляторные пути и вызывает противовоспалительный ответ. Рассмотрены основные молекулярные механизмы, за счет которых симбионтные бактерии препятствуют развитию БА. Представлены результаты клинических исследований, посвященных снижению риска возникновения БА, продолжительности и выраженности симптомов БА при приеме пробиотиков как в детском возрасте, так и во взрослой популяции.
