Резистентность к лептину: возможные механизмы формирования и потенциальные возможности коррекции

Лептин – это пептидный гормон, полученный из адипоцитов и способствующий гомеостатической регуляции
энергетического баланса и обмена веществ (прежде всего жирового) через гуморальный и нервный пути. Лептин,
воздействуя на нейроны в определенных областях мозга, таких как гипоталамус, гиппокамп и ствол мозга,
регулирует потребление пищи, термогенез, расход энергии и метаболизм липидов и глюкозы. Биомаркером
устойчивости к лептину является патологически повышенный уровень циркулирующего лептина, который часто
встречается у людей с ожирением. Резистентность к лептину определяется снижением чувствительности или
неспособностью мозга реагировать на лептин, что сопровождается нарушением способности лептина подавлять
аппетит или увеличивать расход энергии, что в конечном итоге приводит к избытку массы тела, ожирению, другим
нарушениям обмена веществ и сердечно-сосудистым заболеваниям. Резистентность к лептину является важной
клинической проблемой, однако лекарственных средств для ее коррекции еще не найдено и это связано прежде
всего с существенными пробелами в области патофизиологии лептина. В то же время появляется все больше данных
о новых механизмах устойчивости к лептину. В статье представлены данные, полученные в исследованиях,
связанных с резистентностью к лептину и сопутствующими ей заболеваниями, для того, чтобы лучше понять
физиологию и патофизиологию лептина, а также описаны новые стратегии терапии нарушений липидного обмена, в
частности, ожирения.
