Влияние когнитивных нарушений на распространенность и течение гериатрических синдромов

Снижение когнитивных функций (КФ) является одним из основных гериатрических синдромов,
ассоциированных с ранней потерей независимости, увеличением риска развития других гериатрических синдромов и
смертности в пожилом возрасте. Цель – оценить частоту встречаемости снижения КФ и их влияния на
распространенность и течение основных гериатрических синдромов у лиц старше 65 лет. Материал и методы. В
исследовании использована случайная выборка из 611 пациентов в возрасте от 65 лет и старше. Время наблюдения
– 2,5 года. Методы обследования – комплексная гериатрическая оценка. Результаты. Преддементные нарушения
выявлены у 180 (44,4%) участников, деменция легкой степени выраженности – у 91 (14,9%), деменция умеренной
степени выраженности – у 59 (9,7%) и деменция тяжелой степени – у 10 (1,6%). Снижение КФ в пожилом возрасте
ассоциировано с увеличением частоты встречаемости основных гериатрических синдромов – недостаточность
питания, снижение уровня физического функционирования, недержание мочи, депрессия, саркопения и синдром
старческой астении. Динамика изменения КФ ассоциирована с увеличением или снижением риска прогрессирования
остальных гериатрических синдромов. Заключение. У пациентов с когнитивными расстройствами необходимо
проводить дополнительные исследования для выявления других гериатрических синдромов. Также и пациентам с
другими гериатрическими синдромами необходимо проводить обязательную оценку уровня КФ. Коррекция
нутритивного статуса, лечение депрессии и увеличение уровня физической активности являются потенциально
модифицируемыми факторами риска снижения КФ в пожилом возрасте.
