Возраст является надежным предиктором неблагоприятных исходов при остром коронарном синдроме (ОКС). В этом контексте особое значение приобретает главный фактор риска у пожилых пациентов – синдром старческой астении (ССА), или хрупкость. Цель настоящего исследования – разработка математической модели логистической регрессии для выявления ССА у пациентов с ОКС и оценка ее качества в сравнении с другими исследовательскими инструментами, обычно используемыми для комплексной гериатрической оценки пациентов с ОКС. Материал и методы. Для построения математических моделей логистической регрессии использованы данные 300 пациентов c ОКС. У 50 (16,7%) пациентов диагностирован инфаркт миокарда с подъемом сегмента ST, у 126 (42,0%) – инфаркт миокарда без подъема сегмента ST и у 124 (41,3%) – нестабильная стенокардия. Оценка ССА производилась с помощью шкал оценки хрупкости Green и Fried. В ходе исследования осуществлялось построение математических моделей логистической регрессии с применением методов Enter и пошаговых прямых и обратных методов. Результаты. Показатели чувствительности, специфичности и точности при оценке хрупкости пациентов с ОКС при применении модели, построенной с применением обратного пошагового метода, имеют максимальные значения и составляют соответственно 86,3 (80,1; 90,7%), 90,8 (84,9; 94,5%) и 88,4 (86,0; 89,7%). При уменьшении числа измеряемых у пациента параметров с 7 до 4 показатели чувствительности, специфичности и точности ниже и составляют соответственно 81,3 (74,5; 86,5%), 87,2 (80,7; 91,8%), 84,1 (81,5; 85,7%). Заключение. В ходе исследования построены 3 математические модели логистической регрессии, позволяющие производить оценку хрупкости пациентов с ОКС, которые могут быть использованы в условиях неотложной помощи на догоспитальном этапе и после выписки из стационара.
