Приведены данные о ведущих аспектах патогенеза гестационного сахарного диабета (ГСД) в современных условиях. Пандемия ожирения привела не только к экспоненциальному росту частоты ГСД, но и к более ранней его манифестации. Инсулинорезистентность (ИР) на фоне ожирения, поддерживая состояние метавоспаления, рассматривается как основной патогенетический фактор ГСД. Беременность в условиях преконцепционной ИР опасна отдаленными последствиями (в том числе ожирением, жировым гепатозом у потомства), формирующимися за счет механизмов эпигенетического программирования.
