К вопросу гепатотоксичности препаратов, применяемых в дерматологии

Кожные реакции на различные препараты встречаются достаточно часто, тогда как реакции со стороны
печени возникают реже и могут остаться незамеченными. Многие лекарственные препараты способны вызвать
повреждения печени; антиэпилептические препараты, аллопуринол, сульфонамиды, антибиотики и невирапин входят
в пятерку наиболее частых причин лекарственных повреждений печени. Однако не стоит забывать, что список
таких препаратов намного больше и в него попадают лекарственные средства, применяемые в различных областях
медицины. Так, системные ретиноиды на фоне длительного приема вызывают изменения в биохимических показателях
крови, зачастую эти изменения преходящие и не требуют отмены лечения. Циклоспорин, применяемый в терапии
аутоиммунных заболеваний, может вызвать повышение креатинина и острый холецистит. Длительный прием
метотрексата даже в низких дозах или применение системных глюкокортикостероидов может стать причиной
стеатоза и других повреждений печени. При назначении препаратов, способных вызвать нарушения со стороны
гепатобилиарной системы, необходимо учитывать анамнез и сопутствующие заболевания пациента, проводить
регулярный мониторинг биохимических показателей крови и включать в терапию гепатопротективнные средства.
Врач должен принимать во внимание и возможное лекарственное взаимодействие с препаратами, которые пациент
уже принимает, это особенно важно с учетом широкого распространения полипрагмазии. В своей практике мы часто
отдаем предпочтение препарату, содержащему глицирризиновую кислоту и эссенциальные фосфолипиды, поскольку
данное сочетание оказывает не только гепатопротективный эффект, но и обладает противовоспалительным
действием. Включение в терапию с первых дней гепатопротективных средств позволяет избежать нежелательных
гепатотоксичных эффектов, которые в некоторых случаях могут возникать бессимптомно или носить необратимый
характер.
