The paper considers the main impacts of concomitant diseases on the choice of drug therapy for pyelonephritis. Diabetes mellitus, drug addiction, and HIV infection are associated with an increased likelihood of the presence of gram-positive pathogens that are uncharacteristic of non-specific infectious and inflammatory diseases of the genitourinary system, as a result of which the drugs of choice should be highly active against not only gram-negative, but also gram-positive microorganisms. Kidney and liver failure and concomitant drug allergy limit the choice of drug therapy, without affecting the distribution of pathogens. In immunodeficiencies, including iatrogenic ones, the main requirement for the empirical therapy of pyelonephritis is the widest possible spectrum of activity. Along with the features of the clinical course, significant safety limitations of the prescribed drugs are characteristic for pregnant women with pyelonephritis
