ROLE OF MICROASPIRATION OF GASTRIC CONTENTS IN THE ETIOPATHOGENESIS AND PATHOMORPHISM OF BRONCHOPULMONARY DYSPLASIA

A total of 373 neonatal infants on mechanical ventilation were examined. Microaspiration of gastric contents was established from the pepsin isolated from a tracheobronchial aspirate (TBA). In the children with microaspiration, the incidence and severity of bronchopulmonary dysplasia (BPD) in all degrees of gestational maturity were found to be higher and to increase in proportion to enhanced TBA pepsin activity and a shorter gestational period. In this patient category, a classic form of BPD prevailed in preterm infants, BPD was significantly more common than in the comparison group, and a follow-up study revealed a statistically significant number of cases of chronic bronchopulmonary disease developed by 3 years of life.
